The MW4 trophy list has 18 trophies: 8 Gold, 10 Silver and no Platinum. Every one is tied to the campaign, so Multiplayer and DMZ add nothing. The big ones are finishing the story on any difficulty, finishing it again on Realism, reading 50 notes and repairing everything in Base Camp; the rest are mission challenges.
Below is the full list with the mission each trophy belongs to, the difficulty it needs, and a sensible order to earn them. The MW4 campaign has been playable through Early Access since October 16, 2026.
MW4 trophy list: all 18 trophies
The names and descriptions below come from PowerPyx’s trophy list, published October 13 from PlayStation Network data before Early Access began. PowerPyx gives the total as 8 Gold and 10 Silver; the tier for each row is our reading of the trophy icon PowerPyx shows next to it, and those icons add up to the same 8 and 10. Activision has not posted its own list. Mission numbers come from the 16-mission order published by Game Rant and Destructoid.

PowerPyx reports that MW4’s trophies sit as an add-on list under the existing Call of Duty HQ game on PlayStation rather than as a standalone game. Add-on lists cannot have their own Platinum, which is why the total stops at 18. MW4 is not on PS4, so there is only one list, for PS5.
The Xbox and Steam achievement lists have not been published by Activision. Whether they match the PlayStation names one for one is not confirmed.
Which MW4 trophies need a harder difficulty
Four trophies ask for more than the default difficulty:
- Was it worth it? needs a full clear on Realism. Game Rant reports that Realism unlocks only after you finish the campaign once; this is a single media source, and Activision has not described the difficulty modes.
- Can’t touch this needs Veteran or higher.
- Safety’s On and Revenge need Hardened or higher.
Whether a Realism clear also counts for the Hardened and Veteran trophies is not confirmed. If it does, you can attempt those challenges during a Realism run instead of on a separate playthrough.
A simple order to earn every MW4 trophy
- First run on any difficulty. Play for the story and collect notes and yellow crates as you go. This earns Hold the Line and unlocks Realism, according to Game Rant.
- Base Camp. Finish Scavenger by repairing everything there.
- Mission challenges. If MW4 lets you replay single missions, use that for the challenge trophies in the table, such as Bullet Train in Hired Gun and Pacifist in Assassins. If it does not, plan them into a full run.
- Realism run. Clear the campaign on Realism for Was it worth it?. If a Realism run counts for the Hardened and Veteran challenges, it is also a chance at Safety’s On, Revenge and Can’t touch this.
- Notes. Check Noteworthy last. If you are under 50 and mission replays keep your collectible progress, revisit missions with notes you skipped.
Activision has not described MW4’s mission replay options or whether collectibles carry between runs, so steps 3 to 5 depend on how those work once you are in the game.
